    This is a post-war form and relates to 1948 Mental Deficiency Regulations as well as the 1913 and 1927 Mental Deficiency Acts.

    There are two sides to the form. The statutory definitions are on the back

    The 1913 Act said that mental deficiency had to exist "from birth or from an early age".

     

    The following form was printed in 1955 and relates to the 1913, 1927 and 1938 Mental Deficiency Acts, the 1944 Education Act and the 1948 Education (Miscellaneous Provisions) Act.




    In the 1970s, Carla Randle was a nurse in Cranage Hall Hospital, Cheshire (Mental Handicap). One day the hospital threw out its old forms. Someone rescued them from a skip and Carla bought them for 50p. She has a set of several forms which will eventually all be included here.
